A marketing partner built for med spas in Europe and the US that refuse to compete on cheap Botox.
Independent clinic, multi-location group, or luxury aesthetic practice. The structural pressures hitting med spas in 2026 are different on each side of the Atlantic. We work across both. SEO, video content, and consultancy from a Global Search Awards winning agency with European clinic experience and US-market depth.
The structural pain points every med spa faces, on either side of the Atlantic.
Independent, multi-location, or luxury. European or American. The pressure differs by jurisdiction. The structure does not. Most med spa agencies sell tactics. We address what's actually broken first.
Consolidation pressure is squeezing independents on both continents
In the US, AMSPA forecasts 2026 will be the highest M&A year on record for med spas. Over 90% remain independently owned, creating massive PE consolidation opportunity. LaserAway, Ideal Image, and Sona Dermatology are aggressively acquiring market share.
Europe is two to three years behind but moving the same direction. Aesthetic clinic chains are scaling fast in Germany, France, Spain, and the UK. The structural reality is that capital-backed chains can outspend independents on paid acquisition indefinitely. The only sustainable defence is organic search and content authority that paid spend cannot replicate. The independents who win in 2026-2028 are the ones who recognised this two years ago.
Patient acquisition costs have doubled since 2023, and the demand is leaking elsewhere
Google Ads now run $150-300+ per new patient in competitive US metros. Injectable keyword campaigns produce $90-160 per consultation, but the patients are lower value. Treatment plan campaigns produce $2,000-5,000+ patients but require sophisticated targeting most agencies cannot execute.
In Western Europe, the pressure is layered with medical tourism. Turkey, Hungary, and Poland captured significant aesthetic medicine demand from Western European patients at 50-70% lower price points. The European medical tourism market is projected to reach €48.7 billion by 2033. Domestic clinics in Germany, France, the UK, and the Netherlands need to demonstrate clear value beyond price to retain patients who increasingly shop across borders. Paid acquisition is becoming unsustainable for independents on both continents.
Discount marketing destroys the brand it pretends to grow
"$10/unit Botox" ads attract patients who shop on price and leave for the next deal. They drive short-term volume and long-term brand collapse. Premium patients (the ones who spend $5,000-15,000+ annually) explicitly avoid discount-led practices because the signal is wrong.
The fix is value-led marketing that attracts patients choosing you on expertise, outcomes, and trust rather than the lowest number in their Instagram feed. This shift typically increases average revenue per patient by 40-80% within twelve months, with dramatically improved retention. Most agencies will not push you toward this because discount marketing is easier to execute.
Your compliance and YMYL signals are broken, and they are different in every jurisdiction
Google's medical content quality framework (YMYL) was rewritten in 2025 to specifically target aesthetic medicine. Pages require named medical authorship, MedicalBusiness and MedicalProcedure schema, treatment-page depth, and proper FDA-aware claim language. Most independent med spa sites fail all four.
The complexity multiplies across jurisdictions. In the US: FDA jurisdiction (Lemon Bottle not approved, Kybella is), HIPAA on patient communications, state medical board ownership rules, FTC truth-in-advertising on before-and-afters. In the UK: MHRA prescription-medicine advertising rules (you cannot say "Botox" in ads), ASA Code on before-and-after consistency, CMA on pricing transparency. In the EEA: EU MDR device classifications, GDPR data protection, country-specific medical advertising rules. Most agencies pretend the differences do not exist.
Retention is undervalued and 73% of your revenue is at risk
The American Med Spa Association's data shows 73% of med spa revenue comes from repeat patients. Yet most marketing budgets prioritise acquisition. Retaining an existing patient costs virtually nothing compared to acquiring a new one, and lifetime value compounds across years.
The fix is systematic retention infrastructure: automated rebooking reminders triggered by treatment type (Botox at 10-12 weeks, filler at 10-11 months), birthday campaigns, membership programs, and HIPAA-compliant patient communication systems. Most practices have none of this built properly, despite it being the highest-ROI marketing investment available.
The fix is a marketing partner who understands aesthetic medicine across both jurisdictions, not a generalist agency learning on your dime
Most marketing agencies treat med spas like any other local business. They run the standard local SEO playbook and add some Instagram content. The result is mediocre because aesthetic medicine has YMYL signals, regulatory complexity (FDA vs EU MDR vs MHRA), data protection layers (HIPAA vs GDPR), and consolidation pressure that generalists miss. Agencies that specialise in one jurisdiction often miss the other entirely.
Unnamed Project won Best Low Budget SEO Campaign at the Global Search Awards 2025 for our work with a European med spa. The methodology is proven across borders. The case study is documented. The depth is real on both sides of the Atlantic.
Three med spa categories, three different playbooks.
The pressures are universal. The solutions are not. Here's how we work with each category specifically.
Independent and physician-led
Patient acquisition costs are climbing toward unsustainable levels. PE-backed and corporate chains outspend you on paid acquisition indefinitely. In Western Europe, medical tourism leaks high-value patients to Turkey, Hungary, and Poland. In the US, LaserAway and Ideal Image dominate metro markets. The treatment menu is competitive but the marketing infrastructure was never built to compete at this level.
- Local SEO and Google Business Profile optimisation as the highest-leverage acquisition channel
- YMYL signal fixes (MedicalBusiness schema, medical reviewer attribution, jurisdiction-aware content)
- Long-tail treatment-page content that ranks where chains cannot compete
- Retention infrastructure that protects the 73% revenue from repeat patients
- Authentic short-form video that builds trust without production-budget arms races
By month 6, the structural YMYL foundation is in place and local map pack movement is visible. By month 9, the practice is competing for top-three positions on procedure-plus-city searches against larger competitors. By month 12, organic search is meaningfully reducing reliance on paid acquisition, and the retention infrastructure is generating compounding revenue from the existing patient base. The compounding gap with chain competitors starts to close rather than widen.
Multi-location groups
Brand consistency across locations is uneven. Each location's marketing operates in isolation despite operational synergies. Total marketing spend is high but per-location efficiency is low. Patient experience varies between locations in ways that damage the parent brand. Cross-border operators in Europe face additional complexity with country-specific regulations.
- Centralised SEO infrastructure that scales across locations without duplicating cost
- Brand consistency systems (templates, content guidelines, video styles) that maintain quality
- Location-specific landing pages that capture local search intent for each market
- Multi-location Google Business Profile management with strict consistency standards
- Group-wide retention infrastructure with cross-location membership and loyalty
By month 6, every location operates from a consistent brand and content system, with Google Business Profile management aligned across the group. By month 9, location-specific landing pages are ranking on local intent searches for each market, and the group is no longer paying for capabilities it duplicates across locations. By month 12, organic visibility scales with the portfolio rather than being a separate cost per property, and cross-location retention becomes a measurable revenue lever rather than an aspiration.
Luxury aesthetic clinics
The brand is positioned correctly but the marketing infrastructure does not match. Your website looks beautiful but converts poorly. Content production is high-cost and low-yield. The right patients (the $10,000+ annual spenders) are not finding you through search because the SEO foundation was never built.
- Premium-positioned SEO that ranks for high-intent luxury searches
- Treatment-page depth that demonstrates expertise without sounding salesy
- Strategic positioning work that creates real differentiation, not better adjectives
- Email marketing infrastructure that owns the post-treatment relationship
- Selective influencer partnerships with credibility, not pure follower counts
By month 6, the website is rebuilt around real conversion logic rather than aesthetic-only design, and the YMYL foundation is in place. By month 9, organic visibility for premium-intent searches starts compounding, attracting prospects who match the brand positioning rather than discount shoppers. By month 12, the marketing infrastructure matches the brand promise: a website that converts at rates approaching paid channels, a content engine that defends positioning against generic chains, and a retention system that protects the lifetime value of the patients you have already won.
Three services, built for med spas specifically.
SEO, video content, and consultancy. Each one shaped by what actually works in US aesthetic medicine in 2026, not generic agency frameworks.
SEO built for YMYL
Med spa SEO is not generic local SEO. Aesthetic medicine sits squarely in the YMYL category, requiring specific signals most agencies do not understand. Global Search Awards 2025 winning methodology, applied with jurisdiction-aware care for either market.
- MedicalBusiness and MedicalProcedure schema across the site
- Named medical reviewer attribution on every treatment page
- Jurisdiction-aware content (FDA approval status for US, MHRA prescription rules for UK, EU MDR for EEA)
- Treatment-page depth that out-ranks chain practices
- Google Business Profile optimisation as primary local acquisition channel
Video that builds trust
Authentic short-form content that performs on Instagram Reels and TikTok. Production value appropriate to where the content lives. The goal is conviction, not impressions. Compliance-aware on both sides of the Atlantic.
- Treatment education content that demonstrates expertise
- Provider-led video that humanises the brand without overshooting
- HIPAA-compliant (US) and GDPR-compliant (EEA/UK) before-and-after content with proper disclosure
- Patient testimonial structure that converts at the consideration stage
- Compliance-aware scripting that does not create FDA, MHRA, or state board exposure
Consultancy by senior operators
Strategy, brand positioning, marketing infrastructure design. The strategic work that has to happen before any tactical investment compounds. No agency theatre.
- Strategic positioning for practices stuck in commodity pricing
- Retention infrastructure design (memberships, automated communications)
- Marketing channel mix design (organic, paid, retention, referral)
- Multi-location brand consistency frameworks
- Fractional CMO arrangements for groups without full-time marketing leadership
How we won the Global Search Awards with an independent European med spa.
Lana Skyn is a European independent med spa offering Botox, fillers, fat-dissolving injections, laser hair removal, and advanced skincare. Independent ownership, single location, competing against corporate aesthetic chains with significantly larger marketing budgets.
We grew their organic traffic 22x over twelve months. Best Low Budget SEO Campaign at the Global Search Awards 2025. The methodology that won the award is the same methodology we now run for med spa engagements in both Europe and the US, adapted for the regulatory landscape of each market.
Three ways to work with us.
Start where it makes sense for the practice. Move up when results justify deeper investment. We work with med spas at every stage of marketing maturity.

- Two follow-up calls in the 90 days after delivery
The retainer engagement
Ongoing partnership delivering SEO, video, and consultancy across the practice's marketing function. Real partnership, real outcomes.

- Annual brand and strategy refresh cycles
Start with a free med spa marketing audit.
Not a sales call disguised as an audit. A genuine 30-minute working session where we look at your practice's current setup across SEO, YMYL signals, content, video, social, and retention infrastructure, then give you three specific recommendations you can act on this week.
Whether you hire us or not. Whether the audit reveals you need a full agency partner or a single tactical fix. The recommendations are yours either way.
- 30 minutes, fully focused on your practice
- Senior practitioner, not a junior pitch person
- Jurisdiction-aware review (FDA, EU MDR, MHRA, GDPR, HIPAA as relevant)
- Three specific recommendations ranked by impact
- No commitment, no sales pressure, no upsell
- Available 7 days a week across European and US time zones
Five recommendations you can act on this week, with or without us.
Genuinely useful starting points for US med spas at any segment. No paywall, no email gate. If these help, the audit will go deeper.
Add MedicalBusiness schema to your site this week
Most med spas use generic LocalBusiness schema. Google's YMYL framework specifically rewards MedicalBusiness as the primary type, with MedicalProcedure schema on each treatment page. This single change moves rankings on YMYL pages within 60-90 days.
Implementation is a one-week task for a competent developer. The ranking impact compounds for years. Almost no independents have this done properly, which is exactly why it works.
Name your medical reviewer on every treatment page
"Reviewed by our medical team" does not count for YMYL signals. Google needs a named individual with verifiable credentials. Your licensed medical director (MD, NP, or PA) must be attributed by name with full credentials, licensing state, and years of experience.
Create a single author profile page linked from every treatment page with proper Person schema. This change alone moves YMYL rankings measurably within 60 days. Most chains do not have this done properly. The independents who fix it out-rank the chains.
Audit your treatment menu for regulatory status in your jurisdiction
Promoting unapproved treatments creates compounding YMYL damage to your site's quality signals and creates legal exposure. For US practices: Botox, Dysport, Juvederm, and Kybella (submental fat) are FDA-approved. Lemon Bottle, certain Korean lipolytics, and various imported fillers are not, and the FDA has issued enforcement letters. For UK practices: Botox is a prescription-only medicine (POM), which means MHRA rules prohibit naming it in advertising, including Instagram captions and Google Ads. Use "wrinkle-relaxing treatments" or "anti-wrinkle injectables" instead.
For EEA practices: EU MDR classification dictates which devices can be advertised to consumers versus to professionals only. Country-specific medical advertising rules layer on top (France, Germany, Italy each have nuances). If your treatment menu includes off-label or restricted products, the marketing strategy needs to handle them very differently. Most agencies will not flag this distinction. We treat it as a non-negotiable starting point.
Build automated rebooking reminders by treatment type
The single highest-ROI marketing action for most med spas, and it's free. Botox patients are due at 10-12 weeks. Filler patients at 10-11 months. Most practices send generic monthly emails or nothing at all. Treatment-specific automated reminders typically lift rebooking rates by 30-50% within the first 90 days.
Implementation is straightforward if you have a CRM (which you should). The trigger is the patient's last treatment date. The content is a personalised reminder timed to their specific protocol. HIPAA-compliant systems handle this natively.
Calculate your cost per repeat patient versus cost per new patient
Most med spa owners track patient acquisition cost rigorously and ignore patient retention cost entirely. Acquiring a new patient through Google Ads can cost $150-300+. Retaining an existing patient costs virtually nothing beyond the systems to stay in touch.
Run the math for your practice. Total marketing spend divided by new patients acquired versus total retention infrastructure cost divided by patients retained. The ratio is usually 50:1 or worse in favour of retention. Once that number is visible, marketing investment decisions become dramatically easier.
Marketing partnership for med spas that refuse to compete on cheap Botox, in Europe or the US.
We work with a small number of practices at a time. Independent, multi-location, luxury. European or American. The structural pressures are universal. The regulatory landscapes are not. Tell us about your practice and we will tell you honestly whether we are the right fit.
